What Is All-on-4? The Permanent, Fixed Denture Solution Explained

All-on-4 is one of the most requested procedures we perform at New Hope Digital Dentures, and for good reason: it replaces a full arch of missing or failing teeth with a fixed, permanent set that looks, feels, and functions like natural teeth. Here’s what All-on-4 actually involves, who it’s right for, and what to expect if you decide to move forward.

What Is All-on-4?

All-on-4 is a full-arch tooth replacement method that uses just four to six titanium implants to support an entire row of fixed, custom-made teeth. Based on your anatomy, bone support, and health, implants are placed in areas that will allow the proper support and function. This allows us to reach the strongest available bone while avoiding nerves and sinus cavities, which is part of why patients with some bone loss can often still qualify. Once the implants are in place, a full arch of teeth attaches securely to them — no clasps, no suction, no slipping.

Unlike a removable denture that pops in and out, an All-on-4 arch is retained in your mouth.

All-on-4 vs. Removable Dentures

The biggest difference between All-on-4 and a traditional or removable digital denture comes down to stability and bone health.

  • Bite force: Because the teeth are supported by implants in the jawbone instead of resting on the gums, patients chew and function with significantly more confidence than they can with a conventional denture — meaning more of the foods you’ve been avoiding are realistically back on the table.
  • Bone preservation: Implants stimulate the jawbone the way natural tooth roots do, helping prevent the bone loss that causes a denture-wearer’s face to look sunken over time. Traditional dentures sit on top of the gum and don’t offer that benefit.
  • Day-to-day comfort: There’s no adhesive to reapply and no concern about a denture shifting while you eat or speak.
  • Maintenance: Removable dentures typically need relining every two to three years and replacement every five to seven. A well-maintained All-on-4 arch is built to last many years longer, though it still requires regular checkups and cleanings of the implants and prosthetic.

Am I a Candidate for All-on-4?

You may be a good candidate if:

  • You’re missing most or all of the teeth in an upper or lower arch, or your current teeth are failing beyond repair.
  • You’re in good general health to undergo a minor outpatient surgical procedure.
  • You have, or can have, after evaluation, enough jawbone to anchor implants. (Because of the angled placement, many patients who’ve been told they have “too much bone loss” for implants still qualify for All-on-4.)

The only way to know for certain is a consultation that includes a full exam, 3D imaging, and a review of your health history. If All-on-4 isn’t the right fit, we’ll talk through alternatives, including removable digital dentures, so you still leave with a clear plan.

What to Expect: The All-on-4 Process at New Hope Digital Dentures

  1. Consultation and exam – We will take 3D X-rays and CAD/CAM scans of your mouth, review your medical history, and build a treatment plan with a timeline and cost breakdown specific to you.
  2. Vitals and medication – We will review consent forms along with pre-op and post-op information for the treatment and sedation to be completed at your appointment.  We will also finalize any financial arrangements.
  3. Surgery day – Four to six titanium implants are placed in the jawbone. Most patients leave the same day with a fixed set of temporary teeth.
  4. Final, secure attachment – Once the implants have integrated with your bone, your custom, in-house-crafted final arch is attached — fixed in place.
  5. Because our lab technician works in-house and is present during your visits, adjustments happen faster and with fewer return trips than the traditional process.

Recovery Timeline

Healing happens in stages. Initial soft-tissue healing takes about one to two weeks, during which mild swelling or tenderness is normal. The implants themselves need time to fully fuse with the jawbone (a process called osseointegration) before your final prosthetic is placed — typically three to six months. Most patients are back to work and normal routines within days of surgery, and many find the digital, in-house workflow at New Hope Digital Dentures & Implants means fewer follow-up appointments than they expected.
